Guide to Request Procedures for Disclosable Personal Information

For any request by you or your authorized representative to notify the purpose of use, disclose, correct, add, delete, suspend the use of, erase, or stop third-party provision of the personal information we hold (hereafter, a “Disclosure Request”), please follow the procedures below and submit our designated request form.

Where to Submit a Disclosure Request

Please mail the completed request form together with all required documents and fees to the address below.

Documents Required for a Disclosure Request

Fill in every item on the request form in (1) below and send it to us by post. To obtain the form, contact us by e-mail and we will reply with a PDF; please print and complete it.
For identity verification, we will ask you to confirm roughly two pieces of information already registered with us (e.g., telephone number and date of birth).

Disclosure Requests by an Authorized Representative

If the request is made by a representative, please enclose, in addition to the documents in 2.(1), (1) proof of representation and (2) identification of the representative, as described below.

Fees and Payment Method

A handling fee is required only for requests to notify the purpose of use or to disclose information. Please enclose a fixed-amount 
Fee: JPY 1,000 per request

Method of Response

In principle, we will send a written response by postal mail to the address stated on the request form.

Personal information obtained in connection with a Disclosure Request will be used solely to process that request.

We may be unable to comply with a Disclosure Request in the following situations. In such cases, we will inform you of the reason.

Please note that the handling fee is non-refundable even when disclosure is denied.

We cannot verify the identity of the requester or representative.

The submitted documents are incomplete or incorrect.

The requested information is not “disclosable personal information.”

Disclosure may endanger the life, body, property, or other rights or interests of the data subject or a third party.

Disclosure may seriously impede our business operations.

Disclosure would violate laws or regulations.

“Disclosable personal information” refers to personal data contained in a structured filing system over which we have the authority to comply with requests for disclosure, correction, addition, deletion, suspension of use, erasure, or cessation of third-party provision. However, data falling under any of the categories (a)–(d) below are excluded.

(a)

Disclosure of the existence or non-existence of the data could endanger the life, body, or property of the data subject or a third party.

(b)

Disclosure could promote or induce illegal or improper behavior.

(c)

Disclosure could jeopardize national security, damage relations with another country or international organization, or disadvantage Japan in negotiations with them.

(d)

Disclosure could obstruct the prevention, suppression, or investigation of crimes or otherwise hinder public safety and order.
